LTC Criteria

GeriMed Long Term Care Membership Criteria

In order to participate in the entire portfolio of branded and generic group purchasing pricing, pharmacies must be “closed door” and servicing long term care facilities.  These facilities include skilled nursing facilities, nursing homes, assisted living, personal care homes, group homes, chronic psychiatric facilities, correctional facilities, medical at home, and other long term care facilities.

Brand manufacturers have strict criteria for eligibility and GeriMed must adhere to these criteria in order to continue to negotiate favorable contracts for the long term class of trade.

GeriMed members are independent long term care pharmacies servicing only institutionalized and home care patients in a non-retail setting.  When becoming a member, an officer of the company must sign an “Own Use Statement” defining what type patients are serviced and that the medications are dispensed directly to the patient and not sold to a third party or distributor.

Retail independent pharmacies, wholesalers and mail order pharmacies are not eligible for the full GeriMed program, even though they may service long term facilities.  If a retail pharmacy is located in the same building (i.e. under the same roof) as the long term care pharmacy, there may be no connecting doors, windows, or passageways between them in order to qualify for all contracts.  Pharmacies with both long term care and retail services in the same pharmacy should review our  GeriMed Combo program for eligibility.

Manufacturers reserve the right to refuse eligibility of individual members.  GeriMed staff will work with manufacturers to obtain eligibility for members if they follow the criteria for a long term care pharmacy.
